Resumo
Introdução: A lesão por pressão (LPP), é um dano localizado na pele e/ou tecidos moles subjacentes, geralmente sobre uma proeminência óssea ou relacionada ao uso de dispositivo médico. Ocorre como resultado da pressão intensa ou prolongada em combinação com o cisalhamento. O desenvolvimento de lesão por pressão caracteriza um problema de saúde que envolve vários fatores, tanto relacionados ao paciente, quanto ao meio externo, a estrutura física hospitalar, os recursos, os insumos, os profissionais e as condutas da equipe multiprofissional para com o paciente. O conhecimento e entendimento da definição, causas e fatores de risco por parte dos profissionais da saúde é de suma importância, tendo como finalidade de implantar medidas de prevenção e tratamento eficaz. Objetivo Geral: Conhecer os cuidados de enfermagem realizados para a prevenção e tratamento de lesão por pressão em idosos em unidade de terapia intensiva. Metodologia: O presente estudo segue uma abordagem qualitativa sob o método de revisão integrativa da literatura, embasada por (MENDES et al., 2008). O levantamento dos estudos ocorreu em julho de 2021, no portal eletrônico: Portal Regional da Biblioteca Virtual em Saúde (BVS), A técnica de análise empregada para o tratamento dos dados foi a análise temática (MINAYO, 2014), que originou três categorias: Risco de LPP e a atuação do enfermeiro, Tecnologias no tratamento de LPP, Perfil de paciente que evoluem a LPP. Resultados: Evidenciou-se desde o conhecimento técnico-cientifico, formação especifica e capacitação profissional, e carga horaria disponível para o cuidado preventivo, desde a aplicação das escalas e classificação dos riscos é fator primordial para a prevenção de eventos relacionados a assistência à saúde. O reconhecimento do perfil do doente, com maior risco são barreiras que inda precisam ser ultrapassadas para conseguir planejar um cuidado individualizado a cada paciente, além da implementação do processo de enfermagem que garante segurança na assistência prestada ao idoso na terapia intensiva. Considerações Finais: A partir da revisão, pode-se compreender como o profissional enfermeiro, junto de sua equipe, se torna indispensável no contexto do cuidado. Acredita- se que os resultados possam contribuir para reflexões acerca da atuação da equipe de enfermagem no cuidado ao idoso criticamente enfermo, usando o processo de enfermagem como ferramenta principal para o cuidado individualizado.
Abstract/Resumen
Introduction: The lesion by pressure, is a localized damage in the skin and/or underlying soft tissues, usually on a bone prominence or related to the use of medical device. It occurs as a result of intense or prolonged pressure in combination with shear. The development of pressure injury characterizes a health problem that involves several factors, both related to the patient and the external environment, the hospital physical structure, resources, inputs, the professionals and the conduct of the multiprofessional team towards the patient. The knowledge and understanding of the definition, causes and risk factors on the part of health professionals is of paramount importance, aiming to implement prevention measures and effective treatment. General Objective: To know the nursing care performed for the prevention and treatment of pressure injury in the elderly in an intensive care unit. Methodology: This study follows a qualitative approach under the integrative literature review method, based on (MENDES et al., 2008). The development of the review involved six steps: identification of the theme and preparation of the question; establishment of inclusion criteria; definition of the information to be extracted from the studies/categorization; evaluation of the studies included in the review; interpretation of results and; presentation of the review/summary of knowledge. The information extracted from the studies was inserted in a data analysis matrix, in Word, composed by: Title of the article, Referential, Descriptors and Keywords, Research type, Objectives, Results and Conclusions. Results: The searches were performed in the electronic portal BVS. With the exclusion of duplicate articles, which were counted once, 26 studies remained for the reading of titles and abstracts, respecting the objective of the research. Thus, 16 articles were chosen for reading in full because they present a relationship with the theme. From the law reading in full, 08 articles were selected to compose the corpus of the study, as they were in accordance with the criteria and inclusion and exclusion. Final Considerations: The accomplishment of this study was of enormous value for the personal and professional growth, once it amplified the looks in front of the thematic. And, from the review, one can understand how the nurse professional, together with his team, becomes indispensable in the context of care. It is believed that the results can contribute to reflections on the performance of the nursing team in the care of the critically ill elderly, using the nursing process as the main tool for individualized care.
